Jak si správně změřit tlak

Máte vysoký krevní tlak? Nepodceňujte letní teploty. Zatěžují kardiovaskulární systém a především srdce.

Podle posledních odhadů má na celém světě vysoký krevní tlak více než miliarda lidí a kardiovaskulární (srdečně-cévní) onemocnění trápí stále větší část populace. Srdeční infarkt a mozková mrtvice dnes patří k nejčastějším příčinám úmrtí v Evropě. Horké počasí navíc představuje zátěž, s níž se nemocní s rizikovými faktory srdečně cévních onemocnění (např. vysoký krevní tlak, cukrovka, obezita, vysoký cholesterol) vyrovnávají obtížněji než zdraví jedinci.

Nejčastěji jsou ohroženy osoby vyššího věku a lidé s více chronickými onemocněními, mezi které nejčastěji patří choroby krevního oběhu nebo dýchacího systému. Větší pozor by na sebe také měli dávat ti, kteří trpí nějakým metabolickým onemocněním nebo užívají více druhů léčiv.

Jak zvládnout vedro bez újmy – i se zvýšeným tlakem

Základní prevencí je dostatečný příjem tekutin. Ideální je samozřejmě čistá voda. Dáte-li si každou půlhodinku jednu skleničku, budete mít svůj pitný režim pod kontrolou. Při větších vedrech nebo při fyzické námaze příjem tekutin ještě zvyšte, a to zhruba dvojnásobě. Mějte ale na paměti, že nic se nemá přehánět. Nadměrným množstvím tekutin můžete zatížit ledviny i srdečně-cévní systém.

Internista Michal Vrablík k tomu dodává: „Při vysokých letních teplotách platí obecná pravidla a selský rozum. V tropickém počasí by se pacienti léčení s vysokým krevním tlakem rozhodně měli vyvarovat zvýšené psychické nebo fyzické námaze a samozřejmě, jako celá populace, dodržovat pitný režim“.

Vyhýbat byste se také měli přímému slunci, a to především v době mezi 11. a 15. hodinou, kdy jsou jeho paprsky nejsilnější. Ti, kteří nedají dopustit na klimatizaci, by si měli dávat pozor především na nastavení vhodné teploty. Klimatizace sice nabízí příjemné osvěžení, ale v horkých dnech ji nenastavujte na příliš nízko. Ideální je teplota kolem 24 ᵒC. Organismus se hůře vyrovnává s náhlým přechodem z horka do chladu a naopak. Rychlá změna teplot tak může způsobit řadu zdravotních komplikací.

Měření tlaku je třeba provádět správně

Další a velmi důležitou prevencí, jak včas předejít zdravotním problémům, je u rizikových jedinců domácí kontrola krevního tlaku (tzv. selfmonitoring). „Častější měření není v letních měsících obecně nutné, jistě je však vhodné při nově vzniklých obtížích. Může napomoci při rozhodování o dostatečnosti léčby případně při úvaze o nutnosti změn terapie. Domácí měření se však musí provádět podle jasných pravidel a správně, jinak výsledky mohou spíše pacienta stresovat a situaci zhoršovat,“ vysvětluje kardiolog Michal Vrablík.

Ukazuje se totiž, že až jedna ze tří osob se měří nesprávně. Pro domácí měření je proto vhodné zvolit tlakoměr s manžetou, která má větší snímací plochu. Obepíná tak celý obvod paže a není již třeba umisťovat manžetu proti tepně. Jakákoliv pozice je díky tomu možná, čímž manžeta odstraňuje nejčastější chyby při měření krevního tlaku.

 ZÁSADY SPRÁVNÉHO MĚŘENÍ TLAKU

  • Před měřením tlaku krve alespoň pět minut vsedě odpočívejte.
  • Nejméně třicet minut před měřením NEKUŘTE a NEPIJTE ALKOHOL.
  • Tlak si měřte vsedě s opřenými zády a s paží podepřenou o stůl, obě nohy mějte na zemi (nezkřížené). Během měření nemluvte a nehýbejte se.
  • Tlak měřte vždy na stejné paži.
  • Evropská společnost pro hypertenzi zpočátku doporučuje měření po dobu sedmi dní, dvakrát ráno a dvakrát večer. Z těchto hodnot následně vypočítat ranní, večerní a celkový průměr (to vše bez prvního dne). Podrobnější informace na www.zaznamtlaku.cz.
  • Vyhrňte si rukáv – nikdy nedávejte manžetu přes oblečení. Na paži si dejte manžetu tonometru, okraj manžety by měl končit zhruba dva centimetry nad loketní jamkou. Mezi manžetou a paží by měl být prostor maximálně na jeden prst. Předloktí mějte volně opřené o stůl tak, aby manžeta byla v úrovni srdce.
