Prevence rakoviny kůže bude pacientům blíž. Stát letos začne testovat nový screening, do kterého se zapojí část praktických lékařů i nemocničních dermatologických klinik.

Národní screeningové centrum (NSC) dokončilo přípravu další bezplatné plošné prevence, jež má lidem pomoci starat se o své zdraví dřív, než přijde nemoc. Nový screening je zaměřený na rakovinu kůže. Konkrétně na zhoubný melanom a nádory nazývané bazaliom a spinaliom.

Screening bude využívat prvky telemedicíny, strojového učení a umělé inteligence. Cílem je zjednodušit a urychlit práci lékařů a napomoci k odhalení pacientů, kteří mají nádor, ale nevědí o něm. Pokud se na onemocnění přijde včas, pacient má mnohem vyšší šance, že se z rakoviny kůže plně vyléčí.

Koho se prevence týká

Testovací fáze nového screeningu začne v průběhu prvního pololetí roku 2025.

Je zaměřena na screening znamének u žen a mužů starších 50 let se světlým fototypem pleti (I a II), u nichž se onkologické onemocněním kůže nachází nejčastěji.

V pilotní fázi NSC nepočítá, že by se do prevence mohl zapojit úplně každý ve věku 50+. Ministerstvo a NSC pro Vitalia.cz sdělily, že z důvodu minimalizace administrativní zátěže se screening zatím bude týkat jen pacientů registrovaných u lékařů zapojených do pilotního testu.

Celkem existuje šest fototypů pleti. Výše vidíte jen ty, kterých se týká nová prevence.

Kteří lékaři se zapojí

Prevence bude probíhat u vybraných praktických lékařů. Kterých ordinací konkrétně, to zatím není zveřejněno, ale při spuštění pilotní fáze se tato informace objeví na webu screeningu.

Jde o praktické lékaře, kteří budou dohodnuti na návazné péči v některém z dermatologických center, rovněž zapojených do testování nové prevence. Smlouvu chce NSC podepsat s Fakultní nemocnicí (FN) Královské Vinohrady, v jednání jsou také dermatologická centra ve FN Plzeň, Brno, Ostrava nebo Olomouc.

Jak uvádí web NSC, do testovací fáze se zapojí celkem 12 ordinací a šest center. Vybírána budou tak, aby byla rovnoměrně zastoupena po celém Česku.

Jak prevence bude probíhat

Screening by v ideálním případě měl být součástí preventivní prohlídky. 

Dermatologem vyškolená zdravotní sestra (pracující u praktika) nejprve s pacientem povede vstupní rozhovor, aby posoudil, zda splňuje kritéria pro zařazení do screeningu. Pokud ano, se zájemcem vytipuje potenciálně riziková znaménka.

Vyškolený praktický lékař následně provede prohlídku kůže s pomocí chytrého telefonu a speciální aplikace. Poté o vyšetření provede záznam ve screeningové databázi. V případě pozitivního výsledku odešle praktický lékař pacienta na vyšetření do spolupracujícího dermatologického centra, kde dermatolog stanoví diagnózu a nastaví individuální léčbu včetně plánu prevence, uvádí web NSC. 

Otázkou je, proč spouštět screening, když existují aplikace, které mohou lidé používat díky svému mobilu v domácím prostředí. Ostatně bezplatně ji nabízí svým klientům jako benefit Oborová zdravotní pojišťovna (o prvních výsledcích tohoto bonusu jsme psali zde). Na rozdíl od domácího použití bude screening bezplatný, navíc pod dozorem proškolených zdravotníků a s návaznou péčí dermatocenter. NSC pak avizovalo, že program nastaví tak, aby byl pokud možno spolehlivý.

Pilotní fáze screeningu bude trvat do října 2026. Poté Ministerstvo zdravotnictví, pod nějž NSC spadá, ji vyhodnotí. V případě pozitivních výsledků pilotního projektu bude připraven návrh vhodné implementace pro zdravotní systém, říká mluvčí rezortu Ondřej Jakob.
