Padání vlasů a jejich řešení

Vypadává vám víc než sto vlasů denně? Kdo by to počítal. Lidé, kteří trpí alopecií, tedy chorobným vypadáváním vlasů, to snadno poznají i bez počítání. Příčinou je nemoc, stres či léky.

Fyziologicky vypadne sto vlasů denně, což představuje přirozenou obnovu vlasů. Vypadává-li jich více, jedná se o tzv. alopecii. Padání vlasů je několik typů.

Difúzní padání vlasů

Padání vlasů může být difúzní, kdy vlasy vypadávají rozptýleně po celé kštici a dochází k jejich prořídnutí.

Příčiny jsou různé, často to bývá onemocnění štítné žlázy či jiné endokrinní poruchy, vypadávání může být podmíněno nedostatečnou výživou a metabolicky, vzniká v důsledku podávání některých léků nebo chemikálií.

Do této skupiny patří tzv. telogenní efluvium, u kterého větší počet vlasů přejde z anagenní fáze růstu, kdy vlasový váček produkuje vlasové vlákno, do odpočinkové – telogenní fáze. Příčinou jsou nejrůznější formy stresu: horečky, porod, chirurgická operace, hladovění, náhlé vynechání hormonální antikoncepce.

Ložiskové padání vlasů

Další forma vypadávání vlasů je ložisková, kdy se tvoří okrsky vypadaných vlasů, mohou být různě velké a ložisko může být jedno, nebo je jich více, mohou postupně splývat.

Stav, kdy vypadnou všechny vlasy, se označuje totální alopecie, může být spojen i s výpadem ostatního ochlupení, jedná se pak o univerzální alopecii.

Příčina areátní alopecie (alopecia areata) není přesně známa. Dává se do souvislosti s jinými autoimunitními chorobami (vitiligo, thyreoiditida) a alergiemi.

Mužský typ padání vlasů

Jiný typ vypadávání vlasů je tzv. mužský typ, stav, kdy postupně dochází k ústupu vlasové hranice, tvoří se „kouty“ a nakonec vypadají vlasy v rozsahu temene hlavy. Označuje se jako androgenetická alopecie, typická je pro starší muže, ale může se vyskytnout i u žen.

Androgenetická alopecie je geneticky podmíněné onemocnění, jehož podstatou je nadbytek dihydrotestosteronu. Pokud se tedy objeví u mladé ženy, je potřeba pomýšlet na endokrinologickou chorobu.

Stav, kdy vypadávají vlasy v týle, se nazývá ofiáza. Zvláštní forma je jizvící alopecie, která vzniká jako výsledný stav v důsledku různých příčin: poranění, infekce, nádorové onemocnění v oblasti kštice.

Pomoc při padání vlasů

Řešení problému padání vlasů závisí na příčině. Pokud je známa a je možno ji odstranit, je problém vyřešen a vlasy dorostou.

To platí pro telogenní efluvium v případě, že se příčina neopakuje, vlasy dorostou do šesti měsíců a léčba není potřeba.

Po úpravě snížené funkce štítné žlázy nastává postupně růst vlasů.

Vzniká-li alopecie po lécích, po jejich vysazení se spontánně upraví.

Obtížnější je situace u areátní alopecie, v léčbě se doporučují vitamíny ze skupiny B, minerály – zinek a selen, aminokyseliny obsahující síru – methionin, cystein. Lokálně se aplikují kortikoidní přípravky k zevnímu užití v kombinaci s kyselinou salicylovou. Při těžkých stavech se zvažuje celková léčba kortikoidy a u nejzávažnějších forem imunosupresiva. Jejich efekt je však v době podávání, po vysazení může dojít k recidivě.

U mužské androgenetické alopecie se lokálně aplikuje minoxidil. Celkově je možno zvážit podávání blokátoru 5 alfa reduktázy-finasteridu, po jeho vysazení však dochází znovu k padání. U žen se doporučují antiandregeny. Další možností je transplantace vlasů.

  • špatný krevní oběh
  • akutní onemocnění
  • chemoterapie
  • náhlý úbytek hmotnosti
  • horečka
  • nedostatek železa
  • cukrovka
  • poruchy štítné žlázy
  • stres
  • anorexie, bulimie
