Preeklampsie je nemoc, která hrozí jen těhotným ženám. Projevuje se hlavně vysokým krevním tlakem a pokud se neřeší, mohou ženě začít selhávat orgány. Riziko může odhalit screening, jenže o téhle možnosti se moc neví, říká v podcastu 120 na 80 Markéta Řehořová z organizace Nedoklubko.

„Preeklampsie vzniká jen v těhotenství a není na ni lék. Je to nemoc spojená s placentou a pokud se spustí, jedinou léčbou je porod,“ uvedla v podcastu 120 na 80 Markéta Řehořová, členka Výkonné rady organizace Nedoklubko, která pomáhá rodinám předčasně narozených dětí.

Některé ženy se přitom vůbec nemusí cítit špatně. Příznaky preeklampsie jsou snadno zaměnitelné za běžné těhotenské obtíže. Otoky, bolesti hlavy… K tomu se však přidává vyšší krevní tlak a bílkovina v moči, tedy věci, které se kontrolují na pravidelných návštěvách u gynekologa. Nejsou však zase tak časté, aby se jimi vždy dala preklampsie odhalit. Může totiž propuknout i ze dne na den, nejčastěji se tak děje po 20. týdnu těhotenství.

„Nemoc dělíme na ranou a pozdní. Těch pozdních je víc, objevují se po 34. týdnu a většinou se dají lépe zvládat, nedojdou do tak závažných forem, protože miminko je už docela velké a dá se porodit. U těch raných to bývá akutní, preeklampsie může být extrémně rychlá a může skončit i fatálně. Přichází vysoký tlak, který zatěžuje orgány, miminko přestává růst. A vlastně je to takový proces, kdy se všechno zrychluje a zhoršuje,“ říká Řehořová.

Na riziko, že se u těhotné ženy preeklampsie vyvine a bylo by proto dobré ji více sledovat, může poukázat screening, který se provádí na konci prvního trimestru. Není povinný a zdravotní pojišťovny ho neplatí, nicméně většina na něj přispívá. Stojí většinou na jeden až dva tisíce korun. 

Pokud vyjde riziko vyšší, lékař doporučí pacientce užívat aspirin. Až takhle jednoduché to ve výsledku je. Je ale zapotřebí ho nasadit do 14. týdne těhotenství. „Aspirin zlepšuje stav cév. Jsou kvalitnější a víc toho vydrží. Proto je stěžejní ho začít užívat včas, protože po třetím měsíci už jsou struktury vytvořené, už je to všechno postavené,“ uvádí Řehořová.

Podle studií těhotenství žen, které aspirin užívaly, trvalo o měsíc déle než těch, které lék nebraly. A i tento jeden měsíc může pochopitelně hrát ve vývoji dítěte významnou roli. Narodí se větší, riziko různých potíží spojených s předčasným narozením je menší. „Nemoc postupuje pomaleji, tělo není tak zatížené a trvalé následky jsou samozřejmě mírnější. Aspirin má velký smysl,“ dodává Markéta Řehořová. Pro dítě v děloze prášek nepředstavuje riziko.

Ona sama preeklampsii prodělala dvakrát. Při druhém těhotenství jí šlo o život, protože přišel i eklamptický záchvat. V podcastu 120 na 80 svůj příběh popsala, včetně toho, jak se přesně cítila a co následovalo.
