Vědci: Zrak po otravě metanolem není ztracený

Zrak lidí, kteří přežili otravu metanolem, se postupem času zlepšuje, ukázala studie. Vědci zjistili, že zrakové funkce se u většiny pacientů po roce a půl od otravy vrátily k normálním hodnotám. Platí to však jen pro mírně a středně těžkou otravu metanolem.

Výzkumný tým pod vedením Sergeje Zacharova z Kliniky pracovního lékařství 1. lékařské fakulty UK a Všeobecné fakultní nemocnice (VFN) v Praze provádí v určitém časovém odstupu sledování zdravotního stavu pěti desítek lidí, kteří se otrávili při tzv. metanolové aféře v letech 2012 a 2013 (vše ke kauze OTRAVA ALKOHOLEM), kdy bylo ve zdravotnických zařízeních v České republice hospitalizováno více než sto pacientů po akutní otravě, z nichž přibližně osmdesát procent přežilo. Průměrný věk pacientů v době otravy byl čtyřicet šest let.

„K otravě metylalkoholem může dojít nešťastnou náhodou i u člověka, který se napije výjimečně, ačkoliv často ani pravidelně nepije. Alkoholický nápoj mu může nabídnout někdo, komu věří, a tudíž nemá důvod prověřovat kvalitu nabízeného nápoje. S větší pravděpodobností však k otravě dojde u člověka, který alkohol konzumuje pravidelně, ve velkém množství, pije nekvalitní alkohol, alkohol neznámého původu, tedy u člověka, který pije rizikově, škodlivě, případně je na alkoholu závislý,“ říká Petr Popov, primář odd. pro léčbu závislostí VFN. Závislí podle lékaře podceňují varování i příznaky otravy.

Prvními, nespecifickými příznaky otravy bývají opilost, zánět a bolesti žaludku. Varovné signály se někdy objevují až po třiceti hodinách: poruchy zraku, slepota, křeče, po nichž následují bezvědomí a smrt. Slepota může podle Petra Popova nastat již po požití 15 ml metylalkoholu, smrt po 30 až 240 ml. Více v článku Příznaky otravy alkoholem se projeví až po třiceti hodinách.

Podle aktuálních zjištění týmu MUDr. Zacharova se však zrak lidí, kteří přežili mírně a středně těžkou otravu metanolem, postupem času zlepšuje. „Ukázalo se, že se námi měřené parametry zrakových funkcí u většiny pacientů po roce a půl od otravy vrátily k normálním hodnotám. Lidé subjektivně pociťovali, že vidí lépe než brzy po propuštění z nemocnice,“ uvádí lékař. Platí to ale jen pro mírné a středně těžké formy otravy: „U těžce postižených pacientů, jichž byla zhruba pětina, jsme však žádné zlepšení zrakových funkcí nezaznamenali, naopak u některých z nich došlo ke zhoršení,“ doplnil.

Metanol nepoškozuje jen zrak

Poškozený zrak není jediné postižení plynoucí z otravy metanolem. „Kyselina mravenčí vznikající při otravě metanolem poškodila téměř u poloviny pacientů i neurony v hlubokých strukturách mozku (bazální ganglia). Z dlouhodobého hlediska to může vést k určitému poklesu intelektuálních schopností, paměti, poruchy jemné motoriky aj. Chronický neurodegenerativní proces je příznačný zejména pro takzvané civilizační nemoci – Parkinsonovu a Alzheimerovu chorobu, jež v současné populaci postihují čím dál mladší lidi. Vědci nyní zkoumají, zda regenerativní a neurodegenerativní procesy podobné těm, co probíhají v zrakovém nervu v letech následujících po otravě metanolem, jsou přítomny i v hlubokých strukturách mozku,“ objasňují vědci.

Metanol je často přítomen v malých koncentracích v ovzduší pracovišť – v potravinářství, chemickém, farmaceutickém průmyslu atd. Používá se například jako rozpouštědlo, součást barviv, nemrznoucích směsí, pohonných hmot apod. Dlouhodobé zkoumání následků akutní otravy může dát mimo jiné odpověď na otázku, zda vystavení člověka působení tohoto jedu, byť v malých koncentracích, ale po delší dobu, může mít vliv na vznik neurodegenerativních onemocnění (Parkinsonova choroba, Alzheimerova choroba) nebo polyneuropatii (poškození periferních nervů). Vědci budou proto ve zkoumání pokračovat.
